Eligibility

Adult cancer survivors who have completed treatment (surgery, chemotherapy, or radiotherapy) within the past three years.
This program is free of charge.

Screening and Assessment
Use simple standardized tools to assess your symptom distress and weight status.

Provide a Management Plan
Interventions will be arranged according to your level of need (low, moderate, or high).

Refer to NGOs in your residential area to receive personalized support.
Multidisciplinary Professional Support Team

Nurse‑Led Care Services
Manage treatment‑related side effects and provide guidance on healthy living.

Exercise Special Guidance
Provide safe, evidence‑based physical training guidance.

Registered Dietitian Consultation
Provide personalized dietary advice to help relieve symptoms and establish long‑term healthy eating habits.

Psychological Counseling Support
Address fear of cancer recurrence, anxiety, and depressive emotions, and provide psychological interventions.

Programme Overview

Since 2 January 2026, with the support of The Hong Kong Jockey Club Charities Trust, the HKU Jockey Club Institute of Cancer Care has launched a three-year Cancer Care for Survivors Project.
The project aims to implement a community-based cancer survivorship care model in partnership with NGOs to address service gaps across Hong Kong. It also seeks to cultivate champions capable of delivering community-based survivorship care through both professional and non-professional capacity-building initiatives.
