To build a sustainable community support network for cancer survivors who have completed treatment. By connecting survivors with community resources, we provide comprehensive physical and psychological health management and help reduce the risk of cancer recurrence.

Service Targets and Workflow

Eligibility

  • Adult cancer survivors who have completed treatment (surgery, chemotherapy, or radiotherapy) within the past three years.

This program is free of charge.

Screening and Assessment

Use simple standardized tools to assess your symptom distress and weight status.

Provide a Management Plan

Interventions will be arranged according to your level of need (low, moderate, or high).

Referral to Community Resources

Refer to NGOs in your residential area to receive personalized support.

Multidisciplinary Professional Support Team

Manage treatment‑related side effects and provide guidance on healthy living.

Provide safe, evidence‑based physical training guidance.

Registered Dietitian Consultation

Provide personalized dietary advice to help relieve symptoms and establish long‑term healthy eating habits.

Address fear of cancer recurrence, anxiety, and depressive emotions, and provide psychological interventions.

Feeling unwell or uncertain after treatment? Enroll in our programme for assessment and care recommendations, and move toward a better quality of life.

Public Health Talk

Regularly organize talks and workshops to promote cancer‑related information and help the public understand how to support cancer survivors around them.

Peer Support Volunteer

Are you a cancer survivor? We offer training that enables you to use your own experience to become a peer supporter and help other patients on their cancer journey.

This program is free of charge.

Programme Overview

Since 2 January 2026, with the support of The Hong Kong Jockey Club Charities Trust, the HKU Jockey Club Institute of Cancer Care has launched a three-year Cancer Care for Survivors Project.

The project aims to implement a community-based cancer survivorship care model in partnership with NGOs to address service gaps across Hong Kong. It also seeks to cultivate champions capable of delivering community-based survivorship care through both professional and non-professional capacity-building initiatives.
